Only name and photograph is verified in admit card as far as I know ,address is of least importance. So you need not worry. Take your aadhaar card if your name is matching with that of the admit card , even if the photograph is not dicto atleast it

Yes , admission to all the courses is based on the marks obtained in the NIMAHNS entrance exam followed by an interview. Students  have to secure at least 50% marks in the entrance test in order to be eligible for admission.
